Bill Gurley introduced the scientific community's Corrective and Preventative Action (CAPA) process for failure analysis, which involves five steps: detect, investigate, confirm root cause, correct, and prevent. He stressed that skipping root cause analysis hinders effective solutions.
“The scientific community has a process they use when they want to do failure analysis. It's called CAPA: Corrective and Preventative Action. And there are five steps.”
“But you can't skip root cause. That's a requirement. You don't know how to fix it if you don't know actually what happened.”
“CAPA is required by law by the FDA and pharmaceuticals and medical devices, the aerospace industry has regulations that require this type of process if you have a failure.”
